For All Nominations:​

Please explain how the nominee has contributed efforts to the victim services field that are unique, exceptional, or go beyond their job duties (if applicable); how the nominee has demonstrated leadership or teamwork in achieving these efforts; and how the nominee has brought about change for victims or the victim services field that is or will be enduring.

Survivor Activist Award
  • ​Provide context for the nominee's work, experiences, and additional efforts that led you to nominate this person.

  • By their victimization, how has the nominee channeled their experience into a visible contribution to and impact upon the victim services movement?

  • Describe how the nominee's spirit of empowerment is reflected in their enthusiasm in their work within the victim services movement.

Outstanding Student Activist Award
  • ​Provide context for the nominee's work, experiences, and additional efforts that led you to nominate this person.

  • How has the nominee made a visible contribution and impact upon the victim services movement in Pennsylvania?

  • Describe how the nominee's spirit of empowerment is reflected in their enthusiasm in their work within the victim services movement.

Allied Professional Award
  • ​Describe the nominee's history of outstanding service in meeting the needs of victims in collaboration with a Pennsylvania victim service program (non-profit or systems-based program/agency whose primary mission is to assist and/or empower victims of crime).

  • Describe how the nominee's spirit of empowerment is reflected in their enthusiasm in their work in meeting the needs of victims.

  • Describe how the nominee exhibits creativity and innovation in providing services to victims and advocating for the provisions of those services. Please provide two (2) specific examples.

Individual Direct Service Award
  • Provide context for the nominee's work, experiences, and additional efforts that led you to nominate this person.

  • Describe the nominee's history of outstanding service in meeting the needs of individual victims as a direct service provider (i.e. counselor/advocate, victim/witness coordinator) with an eligible program (Pennsylvania non-profit or systems-based program/agency whose primary mission is to assist and/or empower victims of crime).

  • Describe how the nominee's spirit of empowerment is reflected in their enthusiasm in their work in meeting the needs of individual victims and working towards fulfilling the mission of the agency.  

  • Describe how the nominee exhibits creativity and innovation in providing services to individual victims and advocating for their rights. Please provide two (2) specific examples.

Individual Prevention, Education & Outreach Award
  • ​Provide context for the nominee's work, experiences, and additional efforts that led you to nominate this person.

  • Describe the nominee's history of outstanding service in the development and provision of educational programs as a victim service practitioner in an eligible program (Pennsylvania non-profit or systems-based program/agency whose primary mission is to assist and/or empower victims of crime).  

  • Describe how the nominee's spirit of empowerment is reflected in their enthusiasm in their work in meeting the needs of victims and working towards fulfilling the mission of the agency.

  • Describe how the nominee exhibits creativity and innovation in providing education and information about victim services and victim rights within the community. Please provide two (2) specific examples.

Community Services Award
  • Provide context for the nominee's work, experiences, and additional efforts that led you to nominate this person.

  • How has the nominee made a visible contribution and impact upon the victim services movement in Pennsylvania?

  • Describe how the nominee's spirit of empowerment is reflected in their enthusiasm in their work within the victim services movement.

Individual Organizational Capacity Building Award
  • Provide context for the nominee's work, experiences, and additional efforts that led you to nominate this person.

    • Describe the nominee's history of working towards enhancing the capacity or the ability of an eligible program to fulfill its mission of service to victims within the community program (Pennsylvania non-profit or systems-based program/agency whose primary mission is to assist and/or empower victims of crime, i.e. administrative staff, director, board member).

      • Nominee must demonstrate this by:

        • Showing how they are less reliant on government funds.

        • They are ethical and accountable in terms of their program operations, governance (for non-profits), human resources, financial management, and fundraising (non-profit).

  • Describe how the nominee's spirit of empowerment is reflected in their enthusiasm in their work in meeting the needs of victims and working towards fulfilling the mission of the agency. Please demonstrate this by showing the outcomes of the nominee's efforts.

  • Describe how the nominee exhibits creativity and innovation in enhancing the capacity of an organization to fulfill its mission and enhance services to victims. Please provide two (2) specific examples.

Posthumous Award

The nominee must have met the criteria of the individual category specified for which they are nominated (Survivor Activist, Outstanding Student Activist, Allied Professional, Individual Direct Service, Individual Prevention, Education, and Outreach, Community Services, and Organizational Capacity Building).

Program Award
  • ​Provide context for the nominee's work, experiences, and additional efforts that led you to nominate this program.

  • What notable contributions has the nominee made to the field of victim services and beyond?

  • Describe the nominee's history of meeting the needs of the total community.

  • Describe the nominee's reputation for delivering services to the community, i.e. direct services, education, and volunteerism.

  • Describe the nominee's history of stability, longevity, and the ability to sustain over time as an agency/program within the community.

  • Describe how the nominee exhibits creativity and innovation in providing overall services to victims and advocating for their rights as victims. They must demonstrate their creativity and outstanding practices in the agency's/program's administration of their mission. Please provide two (2) specific examples.

  • Describe how the nominee's spirit of empowerment is reflected in their enthusiasm and innovation in meeting the needs of victims and working towards fulfilling the program's mission.

  • Describe how the nominees collaborate with relevant agencies/systems within the community.
